What is a Client Profile?

A Client Profile/Chart is where AutoNotes stores everything related to a specific client, their documents, screening tools, treatment plans, comments, consents, and demographic information.

Why Client Profiles Matter

Client Profiles help you:

  • Keep documentation organized

  • Auto-fill context for future notes

  • Generate treatment plans and summaries more accurately

  • Maintain demographic/clinical data in one place

  • Track progress over time

They are the foundation for accurate AI recommendations and clean documentation.

What You’ll Find in a Client Profile

1. Client Overview (Top Section)

Displays quick info about the client:

  • Name, age, pronouns

  • Active/archived status

  • AI Client Overview (generated automatically after you complete intake/evals/session notes)

This section reminds you that completing documents and marking them as completed updates the client’s AI summary.

2. Tabs Inside the Client Profile

Each tab stores a different type of information.

Documents

All notes created for this client appear here (DAP, SOAP, BIRP, PIE, etc.).

You can:

  • Search documents

  • Sort by newest/oldest

  • Create a new note from the New Note button

  • See whether items are finalized or still in progress

Screening Tools

Displays all screening tools you’ve sent the client, once completed.

You can:

  • Filter by type

  • Send a new screening (“Send Screening” button)

  • View completed results once the client submits them

Treatment Plans

Shows:

  • Active treatment plan overview

  • Any previously generated plans

You can:

  • Generate a new treatment plan

  • AutoNotes will also generate one automatically after you finalize certain notes

Comments

A place to record short internal notes about the client.

  • Comments are factored into the AI Client Overview

  • Only you (and your organization, if applicable) can see them

  • Great for reminders, session context, or clinical details that don’t belong in formal documentation

Consents

This tab stores any consents associated with the client, such as Audio Recording Consent.

Consents appear here once signed.

Face Sheet

The Face Sheet holds core demographic and clinical information, including:

  • Personal information

  • Sex/gender identity

  • Preferred language

  • Race/ethnicity

  • Contact info

  • Medications

  • Allergies

  • Diagnoses (primary, secondary, tertiary)

You can:

  • Update any field

  • Add medications/allergies

  • Archive or delete the client

3. Client List View

On the main Clients page, you’ll see:

  • Every active client

  • Age and pronouns

  • Quick status tag (Active)

  • Search and sorting options

  • Add Client button

This gives you a fast way to jump between profiles.
